I'd go with #3 (or #1).
It is unnatural for me to think that the plugin would change things outside
of the region I currently have selected. The only reason for me to select
a smaller region after doing a scan (barring user error) is so that I can
edit a smaller region of the score.
#4 adds another window I have to navigate to.
#3 gives the most flexibility and draws my attention to the fact that I
have changed the selection region since the results of the scan where
presented. This is a good safety feature. I assume that if the selected region
hasn't changed the user will not have to answer the question of whether to use
#1 or #2...

Since the UI is modeless, it's possible to modify the selected region
after a scan. What's most logical if the region is smaller on "Change
Checked" than the scanned region:
1. Only change the checked accidentals that also appear in the new
region (don't apply anything if there is no region anymore)
2. Change the checked accidentals regardless of selected region
3. If the region has changed, ask the user between #1 and #2. If there's
no region anymore, change all checked accidentals in the document.
4. Control what to do through a setting in JW Plug-in Tool
